I, , certify that I am at least 18 Years of age and that I wish to participate in a La’Movement Fitness class. I recognize that this exercise class may involve strenuous physical activity including, but not limited to, muscle, strength, endurance, cardiovascular, conditioning, and other various fitness activities. I hereby affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or otherwise limit my full participation in this physical class. I do not know of any physical or medical condition that would make it inadvisable or prohibitive for me to participate in the class and I affirm the risk of ANY condition I may have.
I understand that participation in such activities presents risks which could result in physical or emotional injury, death or damage to me. As well as hazards involved with any exercise or fitness activity, such as abnormal changes in blood pressure, fainting, and a remote risk of heart attack, stroke, and other serious disabilities. I agree to accept and assume all the risks connected with my participation in this activity voluntarily, fully understanding of the dangers that physical activity may be hazardous to me and/or my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, THAT MAY BE SUSTAINED BY ME, OR LOSS OR DAMAGE TO PROPERTY OWNED BY ME, AS A RESULT OF PARTICIPATION IN THIS CLASS.
I hereby release, waive, discharge, indemnify and hold harmless La’Movement Fitness LLC, its owners, officers, directors, board members, employees, agents, volunteers, consultants, and all other persons or entities acting on their behalf in any capacity from any and all liability from damages,loss or injuries(including but not limited to death), actions, demands either to person or property, I or any other person may sustain that is in any way related to my participation in La’Movement Fitness classes. I agree not to sue
It is my expressed intent that this waiver and release shall bind any and all members of my family including, but not limited to , my spouse, if I am alive, and my heirs, assigns and personal representatives, if I am deceased. It is also my expressed intent that this waiver and release shall also be deemed as a full release, waiver, discharge, and covenant not to sue insofar as my aforementioned family members, heirs, assigns, and personal representatives are concerned.
I certify that I have adequate liability and health insurance coverage to pay for injuries or damages which are in any way connected with my participation in La’Movement Fitness classes, or I agree to bear the costs of such injury or damage myself.Should La’Movement Fitness LLC, or any acting on their behalf be required to incur attorney’s fees and/or costs to enforce this agreement, I will reimburse them for all such fees and /or costs.
Permission to Use Photography, Video and/or Image
I grant La”Movement Fitness LLC and its affiliates, volunteers,and staff the right to take photographs of me in connection with La’Movement Fitness classes and events. I authorize La’Movement Fitness LLC, its assigns and transferees to copyright, use and publish the same in print and/or electronically.
I agree that La’Movement Fitness LLC may use photographs and/or video of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration,advertising marketing and web content.
I have had sufficient time to read this entire agreement. I understand it and agree to be bound by it
